Other things you may consider bringing with you or sending in advance:

-any lab work done at other facilities or with other doctors

-any records from prior facilities or specialists

-list of supplements/medications (or the supplements/prescriptions themselves) you are currently taking

-any other records, diagnosis, or genetics information you feel would be helpful for the doctor to know or look through
